Whilst the elimination of all risks is largely aspirational, the timely reporting of incidents to all relevant parties is a 'must' do process.
In 2024 the leaving of a timed message (the recipient may not immediately pick-up/respond) is a 'must' do; and call again and again and again until they respond (you get the picture).
Back to my aspirational comment - the sad story in this article is this is now a known risk factor and therefore steps and measures 'must' be put in place to stop/prevent this type of situation happening again.
No matter how good we feel our workspace safety procedures may be, they must be periodically reviewed!
